Job Overview:
An adult day health care (ADHC) center in Glendale is hiring a social worker to screen participants, build and update care plans, and give social services support as part of the multidisciplinary team.
Responsibilities:
- Interview and screen applicants and referrals to see if they fit the full assessment process and the ADHC program
- Refer people who aren't a fit for adult day health care to other services
- Attend and take part in all multidisciplinary team (MDT) meetings
- Build each participant's plan of care around their own goals and concerns
- Complete initial assessments plus quarterly and six-month reassessments, review progress toward goals with the participant, and update the plan of care as needed
- Do additional reassessments when requested by the participant or their representative, after a hospital stay, after a 30-day absence, or after a significant change in the participant's status
- Assess each participant's home environment
- Prepare an individual plan of care for each participant receiving social services
- Provide treatment programs, including counseling and group discussions, as specified in the plan of care
- Lead problem-oriented discussion groups and task-oriented committees
- Refer participants to available community resources as needed
Requirements:
- Master's in Social Work, or Bachelor's in Social Work with work experience
