Job Overview:
A hospice provider is seeking a dedicated Hospice Office LVN to support compassionate patient care in a hospice setting. This office-based, in-person role involves patient care coordination, documentation, and administrative tasks that keep hospice services running smoothly. The ideal candidate has a strong understanding of nursing practices, excellent organizational skills, and the ability to work effectively in an office environment.
Responsibilities:
- Assist with patient assessments, documentation, and care planning in collaboration with the healthcare team
- Manage and update patient records accurately and efficiently
- Coordinate communication among patients, families, and healthcare providers to support quality hospice care
- Support clinical staff with administrative tasks, including scheduling appointments and maintaining medical supplies
- Ensure compliance with healthcare regulations and agency policies in daily operations
- Provide compassionate support to patients and families by addressing questions or concerns about care plans
- Perform other office-based duties as needed to support the hospice team
Requirements:
- Valid Licensed Vocational Nurse (LVN) license with current certification
- Minimum 1 year of hospice experience
- Strong organizational skills and attention to detail in documentation and record keeping
- Excellent communication skills for working with team members, patients, and families
- Ability to work independently in an office setting while managing multiple priorities
- Knowledge of hospice care principles, medical terminology, and relevant regulations
- Compassionate, patient-centered approach to care
